PERSONAL SERVICE AGREEMENT
for
Enforced Chastity
WHEREAS, the parties listed below set forth the terms and conditions under which ladykeykeepers.net will hold the key to a Chastity Device (herein defined) under this Personal Service Agreement, (hereinafter the “Agreement”). The person holding the key to the Chastity Device is designated herein as “Key Keeper.” The person who wears the Chastisy Device is designated herein as the “Muzzled Party.”
DEFINTIONS
Chastity Device: a securely fastened belt-like device that prevents the wearer from engaging in masturbation or sexual intercourse.
Muzzled Party: the party to this Agreement who wears a Chastity Device.
Key Keeper: the person who holds in trust for the benefit of the Muzzled Party all copies of the key(s) to the wearer’s Chastity Device.
Emergency: A situation that presents immediate, physical or mental danger to the Key Keeper, Muzzled Party that in the opinion of the Key Keeper and Muzzled Party constitutes sufficient reason for removal of the Chastity Device. Expression of urgent sexual desire by the Muzzled Party will not be considered an Emergency.
RECITALS
WHEREAS, the effect of the Agreement is that the Muzzled Party will wear a Chastity Device for the defined period of the Agreement;
WHEREAS, the Muzzled Party does hereby warrant and covenant to the Key Keeper that he is in good mental and physical health and does fully, knowingly and with informed consent enter into this agreement;
WHEREAS, the Muzzled Party does hereby state that he wishes to refrain from masturbation and intercourse, and that the Muzzled Party assumes any and all risks and liabilities associated with wearing the Chastity Device;
WHEREAS, the Muzzled Party and the Key Keeper, both affirm, covenant and warranty each to the other that they are over the age of legal majority in the Country or State in which they reside and that they each have authority of law to enter into this Agreement, not being subject to any legal limitation restricting voluntary consent;
WHEREAS, the Muzzled Party agrees to indemnify and hold harmless ladykeykeepers.net and any of its Key Keepers from any harm or injury, mental, or physical, which may be derived from wearing the Chastity Device or entering into this agreement.
WHEREAS, as consideration for this agreement the mutual promises contained herein are the consideration for this agreement, the sufficiency of which is mutually acknowledged and agreed. Any monetary consideration given by the Muzzled Party to the Key Keeper is considered additional consideration and is hereinafter designated as the “KeyKeeping Fee.”
AGREEMENT TERMS
This Agreement sets forth the following terms and conditions:
Special Appointment: I ____________________________, as the designated Muzzled Party do hereby appoint ladykeykeepers.net as the legal custodian of my Chastity Device key.
Term: The Muzzled Party shall initially wear the Chastity Device for a period ONE MONTH (30 days), with the first day of beginning on _______ DAY of ______________________ (MONTH), 200___, and continuing for 30 days ending on __________________ ( DAY) of _____________________(MONTH), 200___. After the expiration of the Initial Term, the key will be returned to the Muzzled Party, by traceable post.
Check One:
Standard Service:
$150/month, which includes one ten (10) minute phone conversation with the Key Keeper. The date of this conversation will take place on __________ ( DAY)________(MONTH), 200____. *This conversation can occur during your incarceration or upon your release. Exact time to be determined later.
No Frills Key Keeping:
$99 per month. You send us the key, we send it back upon completion of your Agreement. Period. There is no additional contact during the month. No exceptions.

1. Termination: This Agreement may be terminated by either party, without cause, upon thirty (30) days advanced written notice in the case of the Muzzle Party and upon five (5) days advanced written notice by Key Keeper to Muzzled Party at the following addresses:

2. Safegarding Key(s): The Key Keeper will employ reasonable efforts to safeguard the Chastity Device key and keep it safe from theft, fire and other loss. However, the Muzzled Party agrees to hold harmless the Key Keeper for any damages arising from the loss of the key for any reason, including but not limited to the negligence of the Key Keeper.
3. Maintenance: The Muzzled Party will maintain the Chastity Device and provide for all expenses of maintaining the device, including replacement of the device. The Muzzled Party will inform the Key Keeper in writing if the Chastity Device breaks or is otherwise non-functioning.
4. Replacement After Emergency : Should the Muzzled Party remove the Chastity Device due to Emergency; when the condition causing removal based on the Emergency is remedied, the Chastity Device will be reapplied by the Muzzled Party under the Key Keeper’s specific instructions of how to proceed.
5. Complaining: The Muzzled Party will refrain from complaining about the discomfort of wearing the Chastity Device or the feelings of sexual desire. Should the Key Keeper determine that excessive complaining is occurring, the Key Keeper has the right to terminate the Agreement with no refund, giving five (5) days advanced written notice. *Online or phone consultations to request early release can be requested at an additional fee.
6. Confidentiality: This Agreement is confidential. Except as may be needed to enforce this Agreement in a Court of Law, the terms of this Agreement shall remain private and will not be disclosed to any non-party to this Agreement without prior written consent.
7. Jurisdiction & Applicable Law: This Agreement shall be governed by and construed in accordance with the laws of the State of New Jersey, and venue shall lie in New Jersey Courts.
